Consumer Information Form Update and Migration to SCTT2012

What's changed?

Infoxchange has upgraded the Client Form inline with the DHS SCTT2012 Consumer Information form. Some fields have changed and data has been migrated across to the new version. There are also some new fields that have been added. The changes for each environment are listed below. There has been less than 5 minor changes to the waitlist client form and these are also listed at the bottom of this page.

Victorian eReferral:

• Employment section added
• DOB Estimate checkboxes replaced (no migration of old to new)
• Carer status dropdown fields updated (includes migration)
• Removal of Maritial Status
• Removal of LGA
• Removal of CALD status
• Country of birth migrated
• Indigenous status updated (includes migration)
• New indigenous origin question
• New Refugee status added with year of arrival if yes
• Removal of main language spoken at home
• Changes to preferred language (includes addition comments which imports old preferred language data)
• Minor change to Communication method to include email
• Removal of GP email
• Contact person section Changes:
  • Removal of Organisation for
  • Removal of YOB
  • Removal of clients carer field
  • Removal of legal decision field
• Changes to Benefits and Insurance section:
  • Heading change
  • Removal of Legal orders
  • New drop down list for Government Pensioner / Benefit Status (includes migration)
  • New Nature of disability field
  • Re-ordering of healthcare card, medicare card and DVA card entitlement as well as inclusion of extra fields (includes migration)
  • Re-ordering and inclusion of extra fields for health insurance status
